Ignore cancelled save-picker results even when they contain a URI, retain read access to saved targets, and exclude save reloads from document-open counts.
Finish recent-list mutations on their existing executor task so shutdown cannot reject a nested refresh. Install the process exception handler once instead of chaining one per manager initialization. Use try-with-resources for PDF copies so failed opens and closes report one result without cleanup null dereferences.
Validation: all three editions build; 48 JVM tests and 107 Android tests pass (one intentional screenshot skip); Spotless and Pro/Lite lint pass. Tests ran on Android 16 and include cancelled save results and repeated exception-handler initialization. Generated Kotlin compiler state is now ignored.
