Add configurable tool telemetry redaction

• Add tool-name and file-path rules to send tool inputs and outputs either as-is or as [REDACTED].
• Apply redaction to tool observations and tool results included in later generation inputs for OpenCode 1 and 2.
• Document configuration and its scope in the README.
