Adds audit/remediation-report.md.

This is a remediation report written in response to the CLI audit by @khushal1512 (commit f014f9f, 22 October 2026). It lists each finding from that audit, what was changed, and how it was tested. The fixes themselves are in #171.

The audit report itself is not part of this PR and will be added separately by its author.

Docs only, no code changes.
