Closes #34.

I wanted the code in good shape before 0.1.0. Seven commits, each passing on its own:

• Unit tests split by area, one class per behaviour, no test importing another. Same tests, still 100% coverage.
• Core tidied: a gone scope is raised in one place in the adapter base, and Replica._apply is split into named steps.
• Integration tests run on every PR against all seven servers, with Cyrus and Baïkal fixtures added. The first run on GitHub passed.
• Fixed the flaky Stalwart tests. Stalwart drops the connection after an early 401 without Connection: close, so the tests now send the login up front. I've reported it to Stalwart and on Retries on connection failures and on a closed idle keep-alive connection python-caldav/caldav#695.
• Stalwart fixture moved to 1.0.1. It refuses a second instance of a uid in a calendar with calendarIds, the same as for a missing calendar, so the JMAP adapter now checks the store before calling it a taken uid. Added Record.server_containers for that.
• mypy and ruff settings in pyproject.toml, the package checked strictly, and the wheel no longer ships tests (2.1 MB to 312 KB).
• Docs trimmed, and the release notes rewritten as one release.

The probes behind the 1.0.1 compatibility rows come in a separate evidence PR, after this one.
